OSPS APEs: Nomination of Beneficiaries
A death-in-service cash lump sum is provided under the scheme and is payable to a beneficiary or beneficiaries at the discretion of the Trustees of the scheme. The following form should be completed by the member and independently witnessed as a guide for the Trustees:
– Member’s wishes concerning disposal of Death in Service lump sum (NOM1) form

It is strongly recommended that this form be completed by the member as early as possible and a further form should be issued in the event of any change in personal circumstances. Please note that the latest form supersedes any previous nomination.
Additionally, the scheme automatically provides dependants pensions. Please refer to the death benefits section for more details. If a member wishes to register a potential dependent other than an automatic beneficiary, please ask them to complete and return the following form:
