Q6: Accidents & Incidents

Question: How does your department ensure incidents are reported, managed and investigated?
Information: Individuals must have been told the whereabouts of the accident report forms.  They should be encouraged to report all incidents, including accidents, fires, and other dangerous occurrences or near misses.  Where incidents are recorded, there should be suitable systems for reporting this in a timely manner to the University Safety Office.  There should also be systems in place to ensure a suitable investigation of the incident to identify both the immediate and underlying causes.  Where corrective action is identified, this should be communicated to the relevant people and actioned accordingly.
