Self assessment of health and safety compliance

As a head of department you are responsible for ensuring the health and safety of those affected by work under your control and for ensuring compliance with University Safety Policies (http://www.admin.ox.ac.uk/safety/ and http://www.admin.ox.ac.uk/uohs/). You should prepare and sign an up to date Departmental Statement of Safety Organisation each year, issue it to your department, and send a copy to the University Safety Office.

You should also be able to give positive answers to the following self-assessment questions, to satisfy yourself that your department is able to meet the minimum requirements outlined and you should ensure that there is auditable evidence of these arrangements. Your departmental or area safety officer (where appointed) will be able to assist you with this. Please contact the Safety Office if you need further assistance.

This document has replaced the Statement of Conformity that was sent to the Vice-Chancellor each year. It is intended to help you better identify areas of poor compliance and rectify them and it will not be used to rank the safety performance of departments.

Please return the Completed form (70kb) to the University Safety Office by 1 April.
